Ok, i see alot of post on this on other sites, so im gonna write my experience with it for future reference. There seem to be many reason why ppl get this, from incorrect blocker patching to incorrect writes to card, well mine came about like this:
I glitched a rom102 rev 106 open, upon successfule glitch i read card, it read fine, but was unknown provider. i saved image, then did some research on it. Some ppl were changing bytes in certain locations and fixing this problem from what i read. i tried that, and couldnt write to card, at first, gave me error. SO here's what i did, i read card, saved image. Opened another image, then reopened the original image, clicked clean codespace (as trying to clean code space without doing so was giving me an error saying there was no clean image for this bin). Then wrote the cleaned codespace image to the card. Wrote fine, AND now had D!sh provider dropped revision to 103 from 106. after that, card programmed fine. Just figured i'd write this incase someone else comes across that error.
so apparently there's more than one cause & fix of this error.

Phreakbytes good post I had almost the same problem, but it was a rev105. Now if I have to glitch into a card I write back a virgin image or the old saved image. Good reason to keep the backup image.
